Integrated Community Learning Disability Team (WDHSCP)

The West Dunbartonshire Health and Social Care Partnership (WDHSCP) Community Learning Disability Team offers specialist support to adults with a Learning Disability, who meet the eligibility criteria. The integrated team comprises health, social work and a range of registered and non-registered services. The Health team comprises:
  • Dietetics
  • Nursing
  • Occupational Therapy
  • Psychiatry
  • Psychology
  • Physiotherapy
  • Speech & Language Therapy
  • Healthcare support workers

Much of the service is delivered on an outreach basis to individual’s homes and /or care placements. In addition a range of health clinics are run such as screening to determine if someone meets the criteria for the Community Learning Disability Team (CLDT) as well as anti-psychotic monitoring clinics and health screening clinics by appointment only once a referral to the team has been accepted. Members of the health team provide assessment and treatment in the community to people with a Learning Disability. They provide specialist support to people with a Learning Disability covering a range of needs including physical and mental health. To access Transition support this would be assessed through the Accessing Adult Social Care Eligibility Criteria which explains that the CLDT provides support to those with critical and substantial need. Further to screening, if required, referrals can be made to community supports offering short term interventions to those who require it. A client contribution is charged for non- personal care social support. There are some charges for activities and/ meals or care alarms
  • The person must live in the geographic catchment area for the team
  • The person requires a confirmed diagnosis of a learning disability
  • The person is aged18 or above for referral regarding mental health and behaviour
